TAILORED MEDICATION - JUST IN TIME

A NEW MEDICATION DISTRIBUTION MODEL FROM HOME TO HOME


In recent years, we have been working with the Department of Clinical Pharmacy & Toxicology from MUMC+ to design a new patient service model to make the care of the future, the care of today.


Medication by Measure focuses entirely on the entire medication process from home to home.


Within the Medication to Measure program, one of the 9 projects focuses on the Medication to Measure-Just in Time distribution model.




THE VISION:


To provide outpatient and clinical integral specialty customized pharmaceutical care that is patient-centered, safe, efficient and effective.


The patient takes control and the healthcare professional provides expertise and support from a coaching role in a team of professionals.




THIS MEANS:


Implementation of the basic concept By Use at Home Medication within the clinic, and where necessary medication from the hospital pharmacy Just-in-Time delivered to the bedside.


By Use Home Medication means the medication that the patient was already using at home before admission, takes it with him to the hospital to continue using during the stay. Upon discharge, the patient also takes this medication back home.


For the outpatient clinic, care around medication is separated from logistics; making possible the delivery/dispensing of medication where and when the patient wants it.


PROJECT APPROACH



1.  Analyzing and translating the right data: to develop innovative and new processes, it is necessary to include all influencing factors. New concepts emerge best when they are designed with the support of the right data and figures. It is important to have the right data AND to place and understand them in their current context.Candide analyzed the right data and translated it into understandable information for the healthcare professional.


2.  Applying new and sometimes already existing/proven technology is of great importance.


Wherever possible, replace manual activities with automated and digitized process steps. This not only improves efficiency, it also allows for better quality and safety.


3.  For this Candide guided the process of implementation in which the right technology is purchased on the basis of fully developed business cases, with the result before, during and after implementation of a well-functioning logistics process in mind.



An example of a new form of medication distribution in the clinic concerns the use of a medication vending machine for medication inventory management and dispensing.



An example in the outpatient clinic concerns a medication collection wall, in particular for repeated prescription medication.
