ZGT, a general teaching hospital with locations in Almelo and Hengelo, is increasingly dealing with elderly people who are in hospital for longer because there is no place available in follow-up care. Read here how we have analyzed the problem and identified possible solutions for ZGT, 3 VVT institutions and Menzis.

In a period of four weeks we interviewed more than 20 stakeholders, analyzed relevant documentation and data and identified bottlenecks in the transfer process. At the end of this period, we presented our findings and a starting point for the short- and longer-term solutions to the directors of the organizations involved. With the help of these results, concrete improvement actions were initiated in 2020.
